Service penetrations were sealed before the electrical first fix was signed off

What happened

On a fast-track programme, fire-stopping around service penetrations was completed and boarded over before the electrical first fix inspection had taken place, because the two trades had been sequenced independently in the programme.

Impact

Several penetrations had to be opened back up to allow inspection, delaying the following trade by a week.

How it was resolved

A hold point was introduced: fire-stopping cannot be closed up until the preceding trade's first fix has been signed off, not just scheduled as complete.

The lesson

Sequencing trades from a generic programme template does not account for MMC's tighter tolerances between activities. Build explicit hold points into the programme wherever one trade's completion is a precondition for closing up another's work.
