Wrong breather membrane spec led to £1m of remedial work

A late supply chain substitution swapped the specified vapour control layer for a vapour permeable breather membrane from the same product family. The delivery paperwork still referenced the original product code, so it was missed at goods in.

A factory witness inspection caught a panel defect before 40 units shipped

A routine witness inspection ahead of a large batch release identified a fixing pattern deviation on the structural panel production line.

Watch out

Service penetrations were sealed before the electrical first fix was signed off

On a fast-track programme, fire-stopping around service penetrations was completed and boarded over before the electrical first fix inspection had taken place, because the two trades had been sequenced independently in the programme.

What worked

A single shared setting-out drawing stopped a foundation misalignment before it happened

The groundworker and the panel manufacturer were each working from their own version of the setting-out drawing, issued a fortnight apart, on a site where the foundation tolerances were tighter than a traditional build.
